FT Lead Outreach Case Manager

LEAD OUTREACH CASE MANAGER

THIS IS A FULL-TIME NON-EXEMPT POSITION, STARTING RATE IS $19-$22 PER HR DOE

Covenant House California (CHC) is a non-profit agency whose mission is to serve homeless and trafficked youth (18-24) offering safety, hope and opportunities to turn their lives around.

Covenant House California’s Oakland site has a 40 bed residential facility in Jack London Square for youth 18-24. We are looking for dynamic Drop-in-Center staff to lead our work as we enhance our daytime services and expand street outreach for homeless youth.

 

We are looking for applicants that have experience working with youth that have experienced homelessness, who have traumatic histories, identify as LGBTQ, are gender non-conforming and/or who often face multiple issues and have complex needs. This person will have knowledge and/or experience working with youth development programs, utilizing a harm reduction framework, using trauma informed practices and will value working with a multi-disciplinary team to create a safe inclusive environment for all.

POSITION SUMMARY: 

 

Under the supervision of the Drop-In-Center Manager, the Lead Outreach Case Manager functions as a lead staff in supporting and developing Outreach services, including collaboration around Street Outreach strategy and direct service provision. Work includes engaging with homeless youth (or otherwise unstably housed) surviving on the streets of Alameda County and in CHC Drop-In-Center.  The position is responsible for working collaboratively with CHC teams, staff and other service providers in the community. The position is also responsible to ensure that documentation, data tracking and reports for Outreach contracts and grants are prepared accurately and timely. The position requires a passion and skill set for positive youth-engagement and comprehensive case management activities including building relationships, linkages, crisis prevention and intervention.

 

ESSENTIAL FUNCTIONS: 

 

  1. Lead strategy, mapping, and direct provision of street-based outreach within Alameda County, providing hygiene supplies, food and resources for homeless youth ages 18 – 24.

 

  1. Lead positive youth engagement and support of day drop-in center for homeless youth (9am – 1pm). Maintain a safe youth-focused environment, support referrals and linkages for homeless youth, and complete case management/program activities as needed.

 

 

  1. Develop and maintain strong collaborative relationships with staff from local and state resources as required by HUD Coordinated Entry. Support agency implementation of Alameda County’s Coordinated Entry assessment tool to aid youth in the admission and application procedures for relevant housing programs.

 

  1. Maintain strong understanding of community housing resources, mental health issues, and state and county resources available for homeless youth.

 

  1. Develop and lead strong community partnerships and stand-alone trainings specific to street outreach needs, including CPR and First Aid, Narcan/Naloxone Administration, and Crisis Intervention/Overdose Prevention. Support agency-integration of harm-reduction strategies, trainings, and protocol to support safety of all youth.

 

  1. Maintain strong understanding of best practices in the field with regard to engagement, outreach and intervention strategies specifically targeting runaway, homeless, and those in danger of sexual exploitation.

 

  1. Develop and maintain strong interventions for program that stem from the framework of trauma informed, harm reduction and positive youth development models.

 

  1. Ensure that all written correspondence, demographic data, contract reports and all other documentation are completed and submitted for review in an accurate and timely manner.

 

  1. Responsible for intake/screening information and reporting as required by program and position.

 

  1. Act as a representative of Covenant House California when attending community meetings or forums on issues centering on homelessness or mental health issues.       Maintain presence at North-County Outreach Collaborative and other community-based meetings as appropriate.

 

  1. Ability to work with funders, donors, board members, media for introduction of street outreach services and van “ride-a-longs” to help increase awareness of homeless youth issues.  

 

  1. Must be a licensed driver qualified to drive a nine-passenger vehicle, and provide transportation for youth to shelter programs or medical treatment.

 

  1. Other duties as assigned.

 

 

JOB REQUIREMENTS: 

 

EDUCATION:                                High School diploma or GED required. BA degree in psychology, social work or a related field is preferred.

 

EXPERIENCE:                                Minimum of 1-2 years of youth work experience is required in addition to having experience working with community based programs for homeless and at-risk youth. Must be a self-starter and team player with the ability to work with other staff members, clients, vendors and the public.

 

 

SKILLS:                                            Must exercise appropriate independent authority when needed. Must have the ability to make independent decisions for client placement. Must be knowledgeable in the areas of crisis intervention, mental illness, and community based service delivery. Must have good organizational skills. Must have excellent verbal and written communication skills.

 

TRANSPORTATION:             Must have reliable transportation and a valid California driver’s license with good driving record.

 

WORKING

CONDITIONS:                               Full Time, must be able to stand for extended periods of time, bend and lift as needed 30-40 pounds.

 

SUPERVISORY:                            N/A

 

 

EQUIPMENT USED:                   Drive the outreach van, computers, fax machines, telephone/cell phone,

and other equipment as required.
