Full Time Case Manager

As our direct care staff work with youth that are 13 - 18 years old, 

we require that applicants are at least 21 years old.


Since 2000, DreamCatcher Youth Services has been the only safe landing for homeless and trafficked at-risk youth in Alameda County who are between the ages of 13-18. DreamCatcher’s comprehensive constellation of programs is designed to serve these youth by connecting them to stable housing, consistent resources, community and peer supports. Over 90% of youth have been through DreamCatcher have moved directly into stable housing, further education or employment.

We believe that basic needs must be met for the work of healing to begin. We are trauma-informed, youth-positive, and strength-based, using a low barrier, harm reduction model. We understand that a core need of all youth is for connection and from the first point of contact, building a trustworthy relationship is our focus. Over a third of our staff are graduates of our program who returned as adults wanting to give back for all they received, for the ways their lives changed.

DreamCatcher provides emergency shelter, a support center where youth can access a community supper, crisis counseling, case management, laundry facilities, hygiene supplies, transportation, computers, academic support, employment referrals, medical clinic, recreation and advocacy, as well as outreach to runaway and homeless youth ages 13-18 throughout Alameda County.

We are seeking qualified applicants to become a part of an exciting and dynamic organization that makes a difference in the lives of homeless and trafficked youth. Applicants that have experience working with youth that have experienced homelessness, who have traumatic histories, identify as LGBTQ, are gender non-conforming and/or who often face multiple issues and have complex needs. Applicants with knowledge and/or experience working with youth development programs, utilizing a harm reduction framework, using trauma informed practices and will value working with a multi-disciplinary team to create a safe inclusive environment would be highly preferred.

DYS is a program serving homeless, disconnected and commercially sexually exploited youth. We offer emergency shelter and provide basic needs for food and shelter as well as comprehensive case management and support services. 

DYS is the only shelter program in Alameda County serving youth 13-18 years of age. Our shelter program has been in operation for 20 years. Soon we will expand our services, increase our beds and provide more in-depth case management and clinical response to these youth. 


The case manager is responsible for working with youth to meet their housing, medical, mental health, employment and educational goals as they move towards finding permanent housing and strong community connections.


  • Engage each youth in developing a Case Plan that addresses their critical needs (housing, education, legal, health issues) through a trauma-informed and positive-youth-development model.
  • Partner with the youth to identify and address their changing needs as they stabilize.
  • Facilitate the client’s access to needed services through referrals and collaboration with community partners.
  • Track client progress and coach them about their role in meeting in their identified goals.
  • Motivate clients to improve wellness in physical and mental health, substance use/abuse/dependence, and other areas that will improve their ability to help themselves.
  • Coordinate among community partners and DreamCatcher services (shelter staff, wellness center, etc.) to meet client needs.
  • Demonstrate positive regard and cultural humility when working with, race, class, culture, country of origin, disability, gender (LGBTQ), immigration status, and language capacity.
  • Collaborate closely with team members to ensure that activities are focused on and in sync with the needs of youth.
  • Maintain all required data and complete reports in a timely manner.
  • Attend weekly supervision with supervisor and all weekly meetings as assigned.
  • All other duties as assigned


Bachelor’s Degree required in Psychology, Sociology, Social Work or a related field.


Preferred three years of professional experience working with at-risk/homeless youth, homeless services, child welfare or serving other at-risk groups.


Excellent communication skills, as demonstrated by on-point, timely, and professional correspondence, both written and verbal inter-agency coordination skills (scheduling appointments, and maintaining collaboration with multiple stakeholders and providers).


 Hours are flexible as needed to serve clients. Some weekends and evenings are required.

Working Conditions

General business/office environment. Must be able to lift up to 40 lbs. on occasion, bend, sit and stand for prolonged periods of time. Some weekend/holiday work may be required.

Equipment Used

Must be able to utilize basic word processing, email, calendar and internet programs. Computers, telephone and other equipment are required.

Knowledge Area

Knowledge and experience working with homeless youth, domestic commercial sex trafficking of minors, male identified youth, and/or additional language skills especially Spanish, is a plus.
